The Problem List module lets you collaboratively manage patient problems (diagnoses). This is particularly useful in settings where there are several clinicians on the team (intern, resident, fellow, attending) or where night float residents typically cover the nights before signing out to the day team. Providers can add or modify a patient’s clinical problems to update other team members who also have a relationship to the patient. In addition, every time a provider enters a charge transaction, order, or clinical note for a patient, the diagnosis codes associated with that item are added automatically to the patient’s problem list. This is a quick way to populate each patient’s problem list, without requiring additional work for the provider.

Problem List At A Glance

The Problem List module opens with the selected patient’s current problems listed in an easy-to-scan Summary.
  • The patient heading row shows the selected patient’s name, age, gender, MRN, and location. Use this information to verify that this is the right patient.
  • The module header shows the number of problems for the patient, such as “Problem List (7).”
  • Each problem is summarized on a separate line with the ICD-10 Code, the Diagnosis Description, and the date of Onset. It may also show the Source of the problem information (the hospital or practice from which it was derived).

Viewing Problem Details

To review the full details of a problem:
  • From the Problem List Summary screen, tap a problem to select it. The Details screen opens and displays the details for that problem which can include date of Onset, date Diagnosed, date Resolved, date Last Used, Type (chronic or acute), Status (active, inactive, or resolved), alternate Codes, indication of whether it is an HCC Diagnosis, and date Created. If your organization has implemented the Health Information Exchange feature and it is enabled in your user profile, you will also see a field for Source (the hospital or practice from which the diagnosis information was derived).
While viewing the details of a problem you can also perform these additional actions:
  • Move to the Details screen for the next problem in the patient’s Summary list. Swipe right to left.
  • Move to the Details screen for the previous problem in the patient’s Summary list. Swipe left to right.
  • Go back to the Problem List Summary. Tap Back .
