| Draft | Standard draft note saved by the provider. The author can edit, discard, or submit/sign it. |
| Pending Upload | Audio recording is captured on the device but has not yet finished uploading to Commure’s backend. |
| Note Generating | Audio has been uploaded and Ambient AI is processing the encounter to generate structured note sections. |
| Note Ready | Note has been successfully generated by the AI model and is available to providers to view, edit, and sign/submit in Pro. |
| Pending EHR Sync | Note has been successfully generated by the AI model, but no matching EHR draft document was found and linked, so the note hasn’t been synced to the EHR. The note remains viewable and editable in Pro. |
| In EHR | Note has been successfully synced to the EHR. Depending on the EHR, the note can be unsigned or signed in the EHR. Providers can still edit it (change template) in Pro, which moves the note back to Note Generating status. |
| MT Expanse Auth Failed | The system failed to authenticate with MEDITECH Expanse during the Expanse document linking process. Note status is set to Pending EHR Sync until the user re-authenticates their MEDITECH account in Pro. |
| Signed | Signed by the author (e.g., resident/mid-level) and awaiting attending co-signature. Routed to the EHR marked as awaiting co-signature, or held in Pro until co-signature completes. |
| Final | Fully completed and signed note, locked against further direct edits. Permanently filed in the patient’s EHR medical record. |
